Private pharmacy antiretroviral therapy distribution: Acceptability and reach of the differentiated service delivery model in Yaoundé, Cameroon

Preliminary findings regarding the implementation of private pharmacy-based HIV service delivery in sub-Saharan Africa indicate that this model may serve as an effective mechanism to further the objectives outlined in the UNAIDS 95-95-95 strategy for the management and control of the epidemic. Biodemographic and clinical data of devolved stable clients in the central region of Cameroon was collected in February 2024 and descriptive statistics analysis was applied. The outcomes included the rates of service utilisation, prescription refill adherence and patient retention and care in private pharmacy settings.

Almost 9% of stable patients undergoing treatment were effectively transitioned from six healthcare facilities to nine private pharmacies, with the reach, statistically higher among females. Almost 85% remained engaged in care following their transfer to community pharmacies. While most clients reported wait time of over 2 hours at public health facilities, almost all clients reported wait time of less than 30 minutes at the private pharmacy. Approximately 69% perceived that the degree of privacy at the healthcare facility was not as protected, while 100% perceived it to be sufficient at the private pharmacy. All clients were satisfied with pharmacy services, with 72% qualifying their satisfaction in the superlative.

Although several studies have captured the practice of private pharmacy dispensation in Africa, the experience is new to Cameroon. Both PLWHA and private pharmacy providers in the country find it acceptable for ART to be provided through private pharmacies. It is slightly cheaper, convenient and largely timesaving for the client. Additional research in other countries with varying political and economic conditions is necessary to fully comprehend the applicability of the results. Finally, community pharmacies promise to be one-stop shops for an integrated package of services including long-lasting injectable ARVs, PrEP and STI care.
